Thursday, March 6th, 2014 from 9:00am to 6:00pm

Friday, March 7th, 2014 from 9:00am to 6:00pm

Saturday, March 8th, 2014 from 10:00am to 6:00pm

It’s important to bring these documents to enroll in Healthcare.gov:

• State Photo ID

• Social Security Cards & Date of Birth (for each person needing health insurance)

• Income Verification (pay stubs, W-2 forms, 2012 taxes)

• Birth Certificate, U.S. Resident Card Passport (or anything else verifying qualifying immigration status)

• Policy numbers of current insurance of those in household

• Current e-mail address & password (We can help you create an e-mail account)

Filing for AN/AI Exemptions

• Tribal Enrollment Card (Village), ANSCA Shareholder Card (Regional/ Village) for each family member OR

• Certificate of Indian Blood (CIB) for each family member

• Social Security Numbers & Date of Birth for each family member

Shingles: a painful skin rash, often with blisters.  The rash usually appears on the face or body and lasts 2-4 weeks.  The main symptom is pain, which can be quite severe. 

Shingles is more common in people whose immune systems are weakened.

If you want give a portion of your PFD, you must go all the way through the Online registration process and it will be on the last page where you can pick the Bethel Family Clinic as the non-profit organization you want to give your donation to. We appreciate any help you can give, to help us better serve you, the rest of the community, and the surrounding villages. If you have already registered and missed out on the opportunity to help the community you can go to the PFD website and there is a green button on the right side in the middle of the page where you can add a donation to help. Remember to go all the way through the online application process in order to Pick.Click.Give. otherwise you will miss the opportunity to help Alaska be a better place to live for everyone.
